Health Care Connect

Do you need a family healthcare provider? Our Ontario government is working to connect every person in the province to primary care. As part of this initiative, we are encouraging anyone without a primary care provider to register with Ontario’s Health Care Connect.

What You Need to Register

If you’re registering yourself, you only need your Ontario health card number and contact information. If you’re registering someone else, you will need their Ontario health card number, current address, and their authorization to
register them.

Registration Steps

Step 1: Scan this QR code to go straight to the first registration
page, or visit: ontario.ca/healthcareconnect and click
the blue “Register Online” button after scrolling down the page halfway.

Step 2: Choose whether you are registering yourself or someone else.

Step 3: Submit your consent for Health Care Connect to use your information to help you find a healthcare provider.

Step 4: Fill in your health card number and basic information.

Step 5: Fill in your contact information.

Step 6: Fill in your health information.

Step 7: Double check the information you provided, and submit.
